The flu jab comes with the cover
Andalucía's public health service will not give an ordinary private-only patient a flu dose, even for payment. A resident who qualifies can buy basic public cover instead, for €60 or €157 a month.
The service is the SAS, and its flu and COVID campaign opens on Thursday 1 October. Ordinary private-only means no SAS entitlement and no public-service mutualist cover.
The convenio especial — paid public healthcare agreement gives a qualifying resident the basic package of public care, which includes vaccination on the official calendar and for risk indications.
It costs €60 a month under 65 and €157 at 65 or over. Confirmed September 2026. We last printed that price on 3 September.

Private cover only: flu goes through your insurer
The campaign instruction Andalucía's health authorities signed on 24 September sends an ordinary private-only patient to the centres their insurer identifies. Nothing in it guarantees that the insurer pays, or that the clinic has a dose when you ring.
An SAS internal note from December 2024, which this year's instruction maintains, says SAS flu doses are not used for ordinary private-only patients with no SAS entitlement or mutualist status, ni siquiera mediando compromiso de pago — not even against a promise to pay.
Two exceptions in the campaign's flu rules apply only to mutualists whose healthcare provider is exclusively private, meaning people covered through public-service schemes such as MUFACE, MUGEJU or ISFAS.
One covers mutualists officially resident in the rural municipalities listed in Annex I of the Convenio Rural — rural agreement, under its conditions.
The other covers school flu jabs in the first two years of Educación Infantil — infant school, with informed consent and no insurer authorisation. Neither covers a non-mutualist adult or child on private cover only.

Private cover only: COVID goes through the SAS
COVID runs the other way. An ordinary private-only patient with an indication gets it at the SAS centres set up for vaccination, through the same channels and on the same timetable as SAS users.
ClicSalud+ — the SAS patient website has a separate route for mutualists and privately insured people to request a COVID appointment, with no card number or NUHSA needed. A NUHSA is the personal number the SAS gives each person on its user database.
At 70 or over, age is the indication. Between 60 and 69 you need another indication: living with or caring for someone in a risk group, health or social-care work, or one of the campaign's listed conditions. For a listed condition, bring a doctor's report that states it.
The convenio especial: who qualifies
Private-only cover does not make you eligible on its own. Qualifying takes three things:
no access to public health cover by any other route;
at least one continuous year of residence in Spain immediately before the date you apply, towards which time lived in another EU or European Economic Area country, Switzerland or the UK can count;
being on the padrón — town-hall residents' list of a municipality in Andalucía when you apply.
The SAS convenio page adds a line for EU citizens and their family members: more than three months' residence is enough for them to join the public health system.
That line is not in the 2013 royal decree the one-year rule comes from, and the page does not say how the two fit together.
A convenio takes effect from its definitive formalisation. The fee buys the basic package, not a flu appointment, and no source we checked says how soon a new holder can book.
The SAS lists the supplementary pharmacy benefit among what the convenio excludes, as we noted on 3 September.
Our 3 April advice was too broad. Royal Decree 180/2026 covers foreign nationals in Spain without legal residence: a provisional document allowing provisional access to care on application, then a card or accreditation after a favourable decision. It is not the route for a legally resident reader on private cover.
Our 26 January and 4 February issues told readers with private cover to register at a centro de salud — public health centre as a backup, and our 14 May issue talked about keeping an SAS card for backstop use. All three left out the prerequisite, which is public entitlement.

Already on the SAS: your date and how to book
Current SAS entitlement, with or without a private policy on top, makes you an SAS user for vaccination. A card or an old number on its own does not prove current entitlement.
Thursday 1 October: children aged 6 to 59 months, pregnant women, care-home residents and staff, and health and social-care staff, among others.
Wednesday 7 October: the general population aged 80 and over.
Wednesday 14 October: the general population aged 70 and over, and adults aged 18 to 59 with a risk condition.
Wednesday 21 October: the general population aged 60 and over.
Confirmed September 2026.
From 75 the flu jab offered is the high-dose Efluelda. A COVID jab needs at least three months since your last dose or infection, whatever your cover.
A group stays open after its date, hasta nueva orden o hasta el final de la campaña — until further notice or the end of the campaign. The date is when requests open; it does not guarantee an appointment that day.
Book through ClicSalud+, the Salud Andalucía — Andalucía Health app or the Salud Responde — Health Responds app, the Salud Responde phone line on 955 545 060, or your centro de salud. ClicSalud+ takes your NUHSA instead of the card number.
A booking alone does not confirm that the SAS holds your correct phone number or entitlement details; check those fields separately.
The instruction announces regional walk-in Wednesdays from 11 November, subject to vaccine supply and campaign data. Plan on booking for now.
An S1 that is not yet registered
The S1 is the form under which the UK or another European state funds your healthcare in Spain, for example as its state pensioner. You register it first with the INSS, Spain's national social security institute; the SAS registration follows.
With current entitlement and a usable SAS record, you can request an appointment from your eligible group's opening date.
Nothing we checked sets a completion time, so submit the INSS registration now; your group stays open after its date. Once registered, your NUHSA books through ClicSalud+ without waiting for the card. It identifies you; it does not prove entitlement.
If you hold private cover meanwhile, indicated COVID vaccination is still available through the separate ClicSalud+ route described above.

Children
Children aged 6 to 35 months with current SAS entitlement, private insurance on top or not, can be booked for flu vaccination from Thursday 1 October through the channels above.
At 3 and 4, children with SAS entitlement may be vaccinated at school, with consent through iPASEN — the regional education system's app for families, or on paper; if not, book at the centro de salud. Which schools take part is not yet clear.
A non-mutualist child with only private cover and no SAS entitlement falls outside the mutualist school exception and goes through the insurer.

The bottom line
An ordinary private-only patient uses the insurer's flu route, and a resident who qualifies can buy basic public cover instead, with no flu appointment attached to the fee. If you arrange your own SAS appointment, request it from your group's opening date.
